Annual Oil Burner Service and Maintenance Plan

In consideration of the sum of $250.00 plus tax, Andrews Oil Co. agrees to provide the following services and component coverage.

Replacement of the following oil burner components and accessories.

Fuel unit	Nozzle adapter
Fuel unit strainer	Pressuretrol
Ignition transformer	Blower motor up to 1/3 HP (belt drive only)
Stack relay	Blower belts
CAD cell relay	Air filter (1 set only during cleaning and special order filters are not covered)
CAD cell eye	Drain expansion tank (during cleaning only)
CAD cell, holder, and leads	Boiler gauge glass, nuts, washers (low pressure only)
Oil burner fan	Air scoop
Oil burner coupling	Boiler drain cap
Oil burner blast tube (non obsolete)	Boilder drain
Oil burner end cone	Purge system (during cleaning only)
Electrodes, porcelains, buss bars and ignition cable	Flue pipe cleaned up to chimney base during cleaning)
Delayed oil valves	Firomatic valve at burner
Thermostat non programmable	Fan-limit control (probe type only)
Emergency switch
Oil burner nozzle
Oil filter complete
Oil filter cartridge

The labor to replace above-mentioned will be covered by service contract.

Water and electrical related services are not covered by the service contract.

For customers who have Co2 detector emergencies, please contact your fire department. Andrews Oil Co. will do a follow-up service call upon request which will be billed at an hourly rate.

GENERAL CONDITIONS

Once per contract year, Andrews Oil Co. will completely tune and service the oil heating equipment covered by service contract.

This service is performed Monday through Friday, 8am to 3pm, excluding holidays.

Andrews Oil Co. exercises reasonable measures of contacting customers to perform the annual cleaning. Therefore, if you do not hear from Andrews Oil Co., please call 528-0766.

Andrews Oil Co. will not refund or give compensation for a missed cleaning.

General Conditions

1. All fuel oil needed during contract period must be purchased from Andrews Oil Co. Delivery by others terminates contract.

2. Only Andrews Oil Co. technicians may service or work on covered equipment.

3. Any repairs or replacements that may be necessary due to damage from hurricanes, tornados, floods, power failures, power surges or reduced voltage, freeze ups, or other acts of God are not covered.

4. Customer agrees to keep work area near equpment accessible and safe for technicians.

5. Environment of equipment must remain in a condition as not to cause premature servicing or replacement of covered components.

6. Calls due to excessive contaminants from fuel oil tank are not covered.

7. Andrews Oil Co. reserves the right to inspect equipment prior to offering service contract.

8. Andrews Oil Co. also reserves the right to not offer renewal of service contract.

9. After hours, weekend and holiday clals will be limited to emergency calls; specifically "no heat". Noise odor and no hot water are not considered emergency calls. Calls of this nature received after normal business hours, weekend, or holidays that cannot wait until the next working day will be answered as promptly as possible at the prevailing hourly rate.

10. Contract is non refundable and non pro-rated.

11. Contract will automatically renew unless notified in writing from customer prior to renewal.

12. Upon receipt of payment for service contract, customer accepts all obligations and conditions of contract.

13. Service call due to the emergency switch "off" is billable. Please check before requesting service!

14. Hot water heaters not included in this agreement.

15. Contract is valid for one year from bill date and in effect when payment is received.

16. Non-contract service calls are billed as follows: any part of the first hour is at full rate and quarterly thereafter.

17. Any service call for no heat due to outside oil tank and/or ourside oil lines that have gelled are billable.

18. Please be aware that plugged boilers due to past due annual maintenance (16 month period) is not covered by this contract.
